Please note that by making a booking for an appointment with Shiatsu Massage Perth you are agreeing to Shiatsu Massage Perth’s Terms & Conditions, and to attend the appointment you have booked.

APPOINTMENTS
You can make an appointment with Shiatsu Massage Perth in a number of ways including online via our website, telephone, text or email. Appointments are available for a variety of durations from 30 minutes to 2 hours.
Upon making a booking, you will receive an email confirming all the important details. You will also receive a text reminder of the booking 2 days prior to your appointment. If this is your first appointment with Shiatsu Massage Perth, you will also receive a New Client Intake Form via email. Please complete this prior to your appointment since this will form the basis for your initial consultation, although if you forget, not to worry, you can fill it out on the day of your first appointment.
CANCELLATIONS & LATE ARRIVALS
If you are late to your appointment, the session will continue with the remaining time available, and will conclude at the original agreed time for the same fee.
A minimum of 24 hours notice must be given to cancel an appointment. Failure to attend an appointment with less than 24 hours notice of cancellation will incur a charge for the full appointment.
An invoice for any cancellation or ‘no show’ charges will be sent to you by email for payment by bank transfer within 7 days. Following a cancellation, payment must be received before any further massage bookings can be taken.
CONFIDENTIALITY
Unless required to do so by law, we will not otherwise share, sell or distribute any of the information you provide to us without your consent.
All shiatsu massage treatments, information and records will be safeguarded by Shiatsu Massage Perth and will remain confidential. Consultations and client treatment details will not be discussed with anyone other than the client, unless the client is under the age of 18 or has a care worker or guardian.
Hard copies of New Client Intake Forms are held in a secure place, adequately safeguarded and not accessible to third parties in compliance with with the practice bound by the Privacy and Personal Information Protection Act (1998) and the Privacy and Personal Information Protection Act (1998) and Health Records and Information Privacy Act (2002).
In order to prevent unauthorised access or disclosure, we employ security measures to protect your information from access by unauthorised persons and against unlawful processing, accidental loss, destruction and damage. We have also put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online. We will retain your information for a reasonable period or as long as the law requires.
Only Amber Queern, the owner and founder of Shiatsu Massage Perth will have access to these records and solely for the purpose of providing you with the most appropriate treatment. Personal and identifiable information will not be shared with any third parties, without your prior written confirmation.
You are entitled to see the information held about you and you may ask us to make any necessary changes to ensure that it is accurate and kept up to date. If you wish to do this, please contact us via email at amberqueern@gmail.com.
HEALTH & SAFETY
The sole practitioner, Amber Queern, at Shiatsu Massage Perth is qualified, and holds professional and public indemnity insurance, for the services that she provides. She also holds a current First Aid certificate.
Due to the nature of massage and the need for close contact, high standards of personal hygiene are maintained and clients are asked to ensure that they also maintain a high standard of personal hygiene.
If Amber Queern is unwell, suffering from a cold or virus, infection or general ill health, she will not be able to work due to the risks of cross contamination. This may result in contacting you to inform you of the issue at the earliest opportunity to make suitable alternative arrangements.
Clients are asked not to attend for a massage if they are unwell, suffering from a cold or virus, infection or general ill health. Please contact Shiatsu Massage Perth via text at 0408 881 972 at the earliest opportunity to rearrange your appointment. If you are unwell and contact us to inform us of your illness, please note that our Cancellation Policy may still apply.
Shiatsu Massage Perth, will not be held responsible for the loss or damage of personal items during your time at the Shiatsu Massage Perth premises.
Clients under the age of 18 years must be accompanied by a parent or guardian. This parent or guardian must give permission for the agreed massage plan to be performed, and will be requested to co-sign the New Client Consultation Form. They will be invited to stay in the room for the duration of the appointment, and expected to remain present throughout the treatment.
Shiatsu Massage Perth holds the right to refuse any treatment if a client is physically or verbally abusive, or demonstrates inappropriate, aggressive or sexual behaviour. Such behaviour may result in a partial or total ban from Shiatsu Massage Perth, and may also result in criminal proceedings.
DISCLAIMER
Shiatsu Massage Perth provides shiatsu massage therapy and exercises or stretches in a variety of forms, for the basic purpose of relaxation and relief of muscular tension and to move and balance the flow of energy through your body, but none of these modalities are a substitute for professional medical care or counselling. At Shiatsu Massage Perth you will not be diagnosed, prescribed medication or medical treatment of any kind, and will always recommend referral to a GP or other qualified medical specialist or practitioner.
The client has a duty to provide accurate and true medical and personal information, to the best of their knowledge, and to keep Shiatsu Massage Perth up to date with details of any medication, symptoms, medical concerns or treatments they are having investigated or undergoing treatment for.
If the client is pregnant and experiencing a low risk or high risk pregnancy according to their doctor or midwife, or if they are currently having or develop complications, the client will discuss the condition with their medical practitioner, and will have a medical release for bodywork signed by their prenatal care provider before continuing with any shiatsu sessions.
A client at Shiatsu Massage Perth may be refused to be treated or treated on part of their body with just and reasonable cause. Shiatsu Massage Perth reserves the right to terminate any appointment at any time if they believe that it is not safe or in the client’s best interest to continue. Likewise, the client has the right to refuse, modify or terminate treatment at any time, regardless of prior consent given.
